A Complete Guide to Choosing Elder Care Services at Home in India

Choosing care for an ageing parent is one of the most important decisions an Indian family makes. This guide walks you through the options, the questions to ask, and what a good elder care arrangement actually looks like at home.

Why elder care at home is rising in India

Joint families are smaller, children often work in other cities, and seniors strongly prefer to stay in familiar surroundings. Home-based care lets elders age with dignity while families stay closely involved in decisions.

Types of elder care services

  • Companion / attendant care: daily living help — bathing, dressing, meals, mobility, medication reminders, companionship. Suitable for relatively healthy seniors who need supervision and support.
  • Home nursing: a trained nurse for clinical needs — injections, IV, wound care, catheter and Ryle's tube management, post-operative recovery, vitals charting. Available in 12 or 24 hour shifts.
  • Specialised programmes: diabetes management, dialysis support, and memory care for dementia and Alzheimer's — usually a mix of a trained caregiver with regular nurse and doctor coordination.
  • Lab and pharmacy at home: blood collection, diagnostics, and medicine delivery so seniors don't have to travel for routine care.

Caregiver vs nurse — how to choose

If the need is mostly daily living help with low medical complexity, a trained caregiver is usually right. If the senior has a chronic condition, recent hospitalisation, or needs clinical procedures, you want a qualified nurse — at least for part of the day.

What to ask before you hire

  1. Is the caregiver background-checked and trained by the agency?
  2. Who is the single point of contact for daily updates and escalations?
  3. How are replacements handled if the caregiver is unavailable?
  4. Is doctor coordination included, or is it an add-on?
  5. What is the notice period and the policy for shift changes?
  6. Are reports and vitals documented in writing for the family?

What elder care costs in India

Costs vary by city and the level of care needed. As a rough guide, a trained attendant on a 12-hour shift typically ranges from ₹18,000 to ₹35,000 a month, and a qualified nurse on a 24-hour shift from ₹40,000 to ₹70,000 a month. Specialised programmes (memory care, dialysis support) are usually priced above standard attendant care because of the training involved.

Red flags to watch for

  • No written agreement, no caregiver background information shared.
  • Reluctance to share nurse qualifications or training certificates.
  • No clear escalation path when something goes wrong at night.
  • Cash-only payments and no formal invoices.
